Fleetwood Shuttleworth longs to give her husband Richard an heir. However, three previous pregnancies have ended in tragedy, and the fourth is proving to be extremely difficult. Moreover, Fleetwood finds a letter among her husband's papers from his doctor, which states that childbirth could be fatal for the young lady. Why did Richard hide this terrible secret from her? To top it all off, the county is rife with rumors of familiars — spirits that supposedly serve evil witches.
One day in the forest, Fleetwood meets Alice, a young but experienced midwife. The herbal infusions that Alice gives her do indeed ease the pregnant woman's condition. Gradually, a sincere friendship develops between the simple girl and the noble lady. Fleetwood hopes that Alice's knowledge and experience will help her and the child survive.
